Regarding Q3, I haven't yet enabled DirSync, but I understand that you must do this before starting a Staged migration. Having done some further reading, it seems that if you are going to decommission an Exchange server after a migration to O365, then a Cutover migration is the way to go. It's a pity that Microsoft hasn't included the ability to choose the mailboxes to be migrated in a Cutover migration, as virtually every Exchange installation must have some redundant mailboxes that need to be kept for historical reasons.
Having to do a Cutover migration raises a couple of other queries.
On most of these SBS2003 installations, there are several mailboxes that are generic, for example enquiries@, which are made available to several users as secondary mailboxes in their Outlook profiles. What would be the best way to handle the migration of these to O365? Possibly change them to Public Folder mailboxes?
Similarly, some of the installations have mailboxes for a different domain and again these are made available to several users as secondary mailboxes in their Outlook profiles. What would be the best way to handle the migration of these to O365?
The O365 Small Business Premium plan includes Office 2013, but obviously a generic mailbox or a mailbox for a second domain wouldn't need the Office 2013 part of the subscription, so is it possible to combine email only and email+Office in the same account?
